At Allen's Family Practice Clinic of Ponchatoula, we strive to meet our commitment to you, to inform you about medical problems, and to treat you with courtesy and respect. PRESCRIPTIONS & REFILLS
Monday: 8:30 am to 5:00 pm
Tuesday: 8:30 am to 5:00 pm
Wednesday: 8:30 am to 5:00 pm
Thursday: 8:30 am to 5:00 pm
Friday: 8:30 am to 3:00 pm
We are closed daily from noon to 1:00 pm for lunch and closed on holidays.
A physician is on call for emergencies 24 hours a day, 7 days a week. If you have an after-hours emergency, please call North Oaks Medical Center at (985) 345-2700 and they will notify the doctor on call.
When calling to speak to the doctor, please give all the important details of your condition to the receptionist. She will forward your message to the doctor, enabling him to respond quickly to your needs. Emergency calls are returned as soon as possible, while other calls are usually returned during the noon hour or after 5:00 pm.
To provide timely service, medical visits are by appointment only. We ask that you assist us in the following ways.
Briefly describe the nature of your problem when making the appointment.
Call several days in advance for appointments.
If you feel you must be seen the same day, notify the receptionist of the nature of your emergency and we will do our best to accommodate you.
If you must cancel and appointment, please call us promptly.
Inform us of any changes in your address or other contact information.
When you are in the office, schedule your next appointment before you leave.
To ensure that you have adequate medication when needed, please refill your prescription long before it runs out. You should call our office at least 48 hours in advance. Also, have your pharmacists contact our office for authorization of your refill.
For new prescriptions, please call our office between 8:30 am and 5:00 pm. Please have the telephone number of your pharmacy available. Remember, we will not refill routine prescriptions after hours or on weekends.
Payment and co-pays are due at the time of service. Allen's Family Practice Clinic of Ponchatoula accepts most major insurances, cash, check, Visa and MasterCard.

FEES, BILLING & INSURANCE
The goal of Allen's Family Practice Clinic of Ponchatoula is to deliver the highest quality medical care to our patients in the most efficient and effective manner. Below is a brief summary of our offices policies concerning payment of fees.
Private Insurance: Payment is due at the time of service for all office charges. After your visit, you will receive an invoice detailing the servcies performed during your visit, the associated charges and the diagnosis. The invoice contains all the information needed to file with your insurance. We also require a copy of your insurance card. For hospitalization, we allow 30 days for your insurance to provide payment. After this time we will request payment of charges by you.
HMO/PPO Patients: If you belong to an HMO or PPO, please inform the receptionist upon your arrival. You will be required to make your co-payment at the time of your visit.
Medicare: We are participating providers in the Medicare program. If you have Medicare and supplemental policy, we will be happy to process your paperwork. However, you will be responsible for paying your deductible and any non-covered charges.
For your convenience, we also accept cash, checks, Visa or MasterCard.
